Patients’ preferences for end-of-life care are defined and easily monitored when they have an Advanced Care Plan (ACP). This includes a will, a durable power of attorney for health care, or (in  some states) a living will. Once the patient has explained that certain actions or procedures should or should not be done in the face of a life-limiting illness, the hospice team can create a care plan that respects the patient’s wishes and focuses on quality over quantity., of the remaining time.

Staff must be able to answer questions free of charge or send the clinic to evaluate the patient and meet with the family. This information will give you a better idea of when it is time for hospitalization.
